Zdravotno-sociálne aspekty pacientov s bronchiálnou astmouSociální vědy - přehledový článek

Posledné tri dekády sa viditeľne v celom svete zvyšuje prevalencia astmy a alergických ochorení, najmä v krajinách so západným životným štýlom, tzv. westernizáciou. Bronchiálna astma vplýva na kvalitu života astmatika a jeho rodiny. Determinanty zdravia takisto vplývajú na vznik a vývoj bronchiálnej astmy. Na vzniku alergie sa podieľajú v 50 % genetické vlohy, zvyšných 50 % tvoria faktory vonkajšieho prostredia. Na vznik a vývoj atopie a astmy vplýva viacero faktorov, ako sú faktory životného prostredia (vnútorné a vonkajšie znečistenie vzduchu, domáce zvieratá, alergény, tabakový dym, hormóny, endotoxíny) a osobné faktory (pohlavie, fajčenie, obezita, potraviny, diéta, dojčenie, rast pľúc). Je potvrdené silné spojenie astmy s nízkou pôrodnou hmotnosťou, nepriaznivým rodinným zázemím matky, ktoré je v interakcii so sociálno-ekonomickým stavom. Na vývoj astmy nepriaznivo vplývajú aj zvýšený príjem sodíka, znížený príjem ovocia, zeleniny a rýb. Priaznivo naopak vplýva príjem kávy (kofeínu) a vitamín C. Expozícia škodlivých látok býva spojená s faktormi zamestnania (profesionálna astma), domáceho bývania a životného štýlu, ktoré závisia od sociálnej triedy. Zistila sa tiež asociácia medzi astmou a nadváhou. Stres matky počas tehotenstva, stres v rodine, úzkostlivosť matky, nedostatočné fungovanie rodiny môžu vyvolať vznik a vývoj astmy. Nedostatočne kontrolovaná bronchiálna astma zvyšuje priame a nepriame zdravotné náklady, limituje astmatikov v práci i v bežnom živote. Preventívne opatrenia a stabilizácia tohto ochorenia sa môžu zabezpečiť aj poznaním zdravotno-sociálnych aspektov pacientov s bronchiálnou astmou.
